I,_______________________________________, agree to be the contact person for a picnic to be held on _________________________, with approximately _______ persons.

A $150.00 deposit is required to reserve the pavilion. In the event of cancellation, a 14-day notice is required for a full refund. If the pavilion rental is cancelled less than 14 days before the picnic, the deposit is non-refundable.

On the day of the picnic, the contact person will advise Perryville Campground when the group begins to arrive so parking or any other needs can be arranged. By 3:00 PM, the contact person will advise Perryville Campground of the total number of persons in attendance so an invoice can be prepared.

The contact person is responsible for the supervision of:
1) Check out/in of all sports equipment provided by Perryville Campground
2) Ground maintenance:  litter pick-up, etc.
3) Entertainment to be monitored--control noise level of music, appropriate, etc.
4) All children under the age of sixteen must be supervised by an adult at all times
5) Swimming pool behavior--hours of use are from 11:00 AM to 6:00 PM

The picnic will begin at __________ and end no later than 9:00 PM.

Perryville Campground will provide as many picnic tables and trash bags as needed. You will have an area set aside just for your use, which will have access to electric and water. Perryville Campground has a convenience store where you may purchase picnic items, food, ice, soft drinks, and many other items. Perryville Campground shall not be held responsible for accidents or loss of valuables on the premises.

The contact person agrees to leave the grounds and buildings in the clean condition they were found in. If not, you will be required to pay a clean up or repair fee of $100.00.
